Patient Consent & Medical Information Sharing Agreement

Attorney review required before production launchv0.1 — DRAFT

By using Docffer as a patient, you acknowledge and agree to the following.

1. Sharing of de-identified case information

You authorize Docffer to share limited, de-identified or admin-reviewed medical information you upload — including imaging, lab results, photographs, written symptoms, and condition summary — with multiple verified doctors in the categories and location radius you select, so they can submit treatment offers.

2. Hidden identity

Your name, email, phone, exact address, date of birth and identity documents are hidden from doctors until you select or authorize a specific doctor according to platform rules.

3. Not a substitute for in-person care

Treatment offers reviewed remotely are not a substitute for an in-person examination. Doctors may indicate that an in-person exam is required before any treatment.

4. Independent professional judgment

All medical opinions, recommendations and treatment offers are the independent judgment of the doctor providing them, not Docffer.

5. Upload responsibility

You should not upload information you do not want shared with verified doctors who match your category and location criteria.

6. Beta pricing notice

Docffer may introduce optional paid features later. You will never be charged without an explicit checkout consent.

7. Withdrawal

You can withdraw a case from the marketplace at any time from your dashboard. Data deletion is handled via the Data Deletion Request page.
